FEES FOR SERVICES:
Fees are discussed before treatment begins and are payable at the time the service is provided.
The first consultation is free.
You are responsible for finding out if your Health Insurance Company can reimburse you for my services.
If so, I will supply you with the necessary documentation.
CONFIDENTIALITY:
Services provided are strictly confidential and in accordance with State and Federal Law. Information will be released, only with the client's written authorization.
LOCATION:
We are located on South West Street, between 4th and 5th Street and West of Lafayette Street.
(Free parking is available for two hours in the municipal parking structure at the corner of Lafayette and 5th Street.)
